The CareConnections Web Site (the "Web Site") is designed to help you learn more about the rehabilitation management services of CareConnections. Our privacy policy explains how we may collect, use, and aggregate personal information about you on this Web Site. CareConnections respects your personal privacy and is committed to protecting it.

Health Insurance Portability and Accountability Act (HIPAA)

CareConnections meets the criteria for a Business Associate under the HIPAA regulations. Subscribers will be provided with a Business Associate Addendum to supplement our base CareConnections Provider Agreement.
Under the HIPAA privacy rule, covered entities may disclose protected health information regarding their patients for health care operations. This includes disclosures to CareConnections that help the subscriber evaluate the performance of their staff therapists and the overall effectiveness of their services to facilitate improvement and become more efficient with care.

Your Acceptance Of These Terms

By using this site, you signify your acceptance of the terms of the CareConnections Privacy Policy. If you do not agree to the terms of this Privacy Policy, please do not use the CareConnections site and exit the site immediately. Your IP Address

For each visitor to the CareConnections site, Therapeutic Associates' servers automatically collect information about which pages are visited and the IP address or domain name of visitors. We use your IP address to help diagnose problems with our server and to administer our Web Site. Your IP address is used to help identify you and to gather broad demographic information. This information helps us better tailor your personalized experience on the Web Site.

In this way, e-mails that you receive from CareConnections, which may reflect specific medical information of interest to you, cannot be linked to you. If you access CareConnections through an e-mail or Internet account maintained by your employer, please note that it is possible that your employer may monitor your e-mail or Internet communications.

Notification of Changes

This Privacy Statement may be revised over time as new features are added to the Web Site or as Internet security and privacy standards evolve.
