What is CareConnections?
CareConnections is a Web-based suite of rehabilitation medical management services.
CareConnections uses combinations of tools to assist providers and payers with the medical management
of patients and clinical services. These tools are the:
- CareConnections Outcomes System a functionally based patient data
collection system that compares clinical outcomes to similar groups of clinics
and provider submitted episodes of care. Outcome reports provide valuable
clinical information and management data that can be used to promote best
practices, improve quality of care, and add value for those responsible for
the costs of care.
- CareConnections Patient Satisfaction Monitoring standardized survey
for patients, allowing them to provide valuable feedback about their provider
experience. All results can be submitted for benchmarking against therapy
sites from around the country.
- CareConnections Treatment Guidelines therapist written and researched
parameters of care grouped by anatomical body region and cross referenced
by diagnosis. Each guideline suggests appropriate evaluation tests and measures,
appropriate functional goals, appropriate therapeutic modality and procedure
interventions, discharge criteria, and considerations for additional visits.
- CareConnections Prior Authorization/Advice System or “PAS” PAS
ties it all together as a tool that permits 24/7 Web-based dissemination
and acquisition of utilization management information. PAS delivers prospective
utilization of care in the form of an appropriate number of visits per specific
patient condition along with an appropriate CareConnections Treatment Guideline.
PAS provides concurrent utilization management based upon initial measures
and continued monitoring using the CareConnections Outcomes System and provider
requests for additional treatment. It also provides retrospective utilization
management in the form of CareConnections Outcome reports and benchmarked
utilization statistics.
