Based on past and present feedback from CareConnections clients, we learned that patients sometimes had difficulty with the Pain and Perceived Improvement scales due to the placement of the wording under the actual scale. For example, the pain scale had the words “No Pain” aligned below the left side of the pain scale and the words “Worst Pain Imaginable” aligned below the right side of the pain scale. This sometimes caused confusion for patients in determining where to mark on the scale since the captions ran under the index line.
Based on this feedback and on suggestions from multiple providers using the CareConnections Outcomes tool, we have done a minor revision of the placement of the captions below the pain scale. Captions are now placed directly to the left and right of the Pain and Perceived Improvement scales.

All forms on the CareConnections website have been replaced with updated versions of the Pain and Perceived Improvement scales. We recommend that you begin using the new version of the forms as soon as you have exhausted your current copies of the CareConnections outcomes tool. New forms can be downloaded by logging into your CareConnections account and selecting the “Patient Worksheets” link.
As a reminder, please be sure to disable all page scaling options in Adobe Acrobat Reader when printing outcomes forms. Failure to turn off page scaling will result in your Pain and Improvement indexes measuring short of 10 cm. It is always a good idea to verify that your lines measure 10 cm before making copies and giving them to patients.
